Forms
The Forms Module allows you to access active forms and view and manage submitted forms.
How to Navigate the Forms Page
Log In – Make sure you’re signed into your ChurchPad account.
In the left‑hand navigation bar, click Forms. This brings you into the Dynamic Forms area.
Switch Between “All Forms” and “My Submitted Forms”
At the top of the page, you’ll see two tabs:
All Forms – every active form your church has published.
My Submitted Forms – the subset you have already completed.
Click the tab you need to toggle between: Tabs: All Forms | My Submitted Forms

Browse the Forms List
The table displays four columns:
Form Name – Title of the form (e.g., “Baby Shower,” “Baptismal”).
Description – a short blurb explaining its purpose.
Expired Date – when the form closes (or “No Expiration Date”).
Action – the “⋯” menu for form‑specific actions.
Use the search bar to find a specific form, or use the pagination controls at the bottom to navigate through the pages of forms.
View or Fill a New Form
In All Forms, click ⋯ → View to open a blank form.
Complete each question (text, radio, checkbox).
Required fields are marked with a red asterisk.
Click Submit at the bottom.

Review Your Submitted Responses
Switch to My Submitted Forms.
Locate your form, then click ⋯ → View Response
Scroll through to see your answers.
Edit a Submitted Form
If your church allows updates:
While viewing your submitted response, look for the Edit icon (pencil) in the lower‑right corner.
Click the Edit icon to reopen the form in edit mode.
Update any answers as needed.
Click Submit again to save your changes.
Note: Once the form’s expiration date passes, you’ll no longer be able to edit.
